Clinical Pharmacist, Community Pharmacy Programs - Fraser Health Community Fraser Valley Salary: CAD $51.37 - $64.19 / hour (Full‑time) Job Summary We have a full‑time prospect for a Clinical Pharmacist to provide pharmacotherapy services to community dwelling clients in Abbotsford, BC. Responsibilities - Consult with physicians, nurses, and other health care providers to develop therapeutic plans for each client. - Individualize client medication therapy by reviewing clinical data and modifying plans as needed. - Provide medication counselling to clients and families, demonstrating techniques and assessing understanding. - Assist the health care team in developing drug therapy guidelines and monitor guideline effectiveness. - Participate in client home visits, program team rounds, and meeting participation as needed. - Act as a program resource person, providing detailed drug information and supporting staff. - Provide academic detailing services to physicians to optimize prescribing practices. - Contribute case studies and review articles to professional journals. - Provide instruction and/or supervision to pharmacy students and residents. - Participate in quality improvement activities and medication safety initiatives. - Manage medication inventories and report procedural errors. - Prepare reports on various activities and maintain electronic records. - Perform other related duties as assigned.
