Description
Integrated Home Care uses a strength‑based Client Centered Case Management practice framework. As a Registered Nurse you will develop care plans, provide direct nursing care, coordinate services, and perform assessments in clients’ homes or alternate care settings. You will work mobile, traveling within Airdrie and surrounding rural areas. Responsibilities - Develop and implement individualized care plans based on client strengths and needs. - Provide nursing assessments, case coordination, and direct care in partnership with clients and families. - Collaborate with a team‑based approach, working remotely and providing care in community settings. - Operate laptop and mobile phone to document care and manage logistics. - Perform manual handling duties, such as lifting, carrying, and moving patients up and downstairs, lifting approximately 30 pounds routinely. - Participate in training and learning resources to perform work safely. Qualifications - Completion of an accredited nursing education program. - Active or eligible registration and practice permit with the College of Registered Nurses of Alberta (CRNA). - Current Basic Cardiac Life Support – Health Care Provider (BCLS‑HCP) certification. - If not practiced as an RN within the past five years, completion of CRNA Medication Management (Chapters 1‑4) and NextGenU Health Assessment Resources Certification before employment. Additional Required Qualifications - 2–5 years of recent acute‑care experience. - Prior Home Care or Case Management experience. - Experience with surgical wounds, drains, CVAD, HPTP, and palliative/end‑of‑life care. - Experience with community‑based and long‑term care settings. - Canadian Nurses Association certification in Gerontology (preferred). - Experience with Home Parenteral Therapy, wound/skin and ostomy care, palliative support, and client teaching. Preferred Qualifications - Previous Home Care and community care experience. Pay and Perks Provisional permit holders: $40.76 – $55.79 per hour; upon obtaining a full practice permit, rate adjusted to the applicable Registered Nurse rate.
