A Social Worker for Integrated Care Solutions will be able to adapt social work principles and practices to the community setting. This includes, but is not limited to, assisting the community adult client to maximize social, emotional and cognitive functioning that promote optimum independence and improve quality of life.

Person-and family-centered care (PFCC) Person-and family-centered care (PFCC) is an approach to care that recognizes the importance of family in a patient’s life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. This approach to care provides respectful, compassionate, culturally safe, and responsive care that meets the needs, values, beliefs, and preferences of the patient, their family, and others identified as significant to their life from diverse backgrounds and settings.

Full‑time Position Available: Requires Monday to Friday Daytime availability (approx. 30 visits per week).

Home Care Position: pays a per visit rate and mileage in-between clients, as well as a separate virtual visit rate.

Areas of Service: North Simcoe Muskoka region.

Duties and Responsibilities

Communicate with clients, families/caregivers, service providers and other professionals to collaborate and coordinate services

Maintain and manage a caseload in an efficient and effective manner ensuring priority code timeframes are met

Assess the client’s adjustments to medical, emotional, psychological or social problems and the factors that affect them

Utilize a client-centered approach to identify, prioritize and monitor potential psychosocial problems and goals

Establish, implement, evaluate and adjust the psychosocial treatment plan in collaboration with the client and/or family, caregivers, care coordinator and other community partners

Enhance or teach coping skills e.g. relaxation, stress management, anger management, healthy grieving

Advise clients/caregivers on the availability of community resources and contact agencies on their behalf when appropriate e.g. Federal, Provincial, Municipal and Local agencies in relation to income security, pensions, financial assistance, etc.

Determine when further social work intervention in no longer necessary and assist clients in planning for discharge

Recommend and facilitate referrals to support services within the community

Developing a flexible, responsive system to meet changing and competing demands

Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body

Liaise with other Case Managers, funders or Government&hospital programs as required

Liaise with Provincial regulating body as required

Complete other tasks as requested

Requirements

Be an active member in good standing of the Ontario College of Certified Social Workers and Social Service Workers

Possess general computer skills and have access to a computer with appropriate password protection and internet capabilities

Minimum of two (2) years of community-based therapy experience (preferred)

Excellent command of the English language, both written and oral (French considered an asset)

Be able to drive, hold a current, valid driver’s license and have access to a vehicle in good operating condition (preferred)

Provide a police clearance (Vulnerable Sector Check) that meets the Bayshore Home Health policy

Hold a current CPR certificate (CPR-C or BLS and is only acceptable a year after issue)

Possess strong work ethic with commitment to regular attendance
