Key Responsibilities:• Provide detailed support to vulnerable tenants • Implement and review individualized support plans • Conduct weekly visits and increase support during transitions • Offer guidance on managing responsibilities effectively • Ensure accurate record-keeping of tenant interactions

Requirements:• Background in housing support or social care • Knowledge of community integration practices • Excellent interpersonal communication skills • Capacity to manage a diverse caseload • Access to a vehicle for site travel
