Elevate patient care as a Pharmacist dedicated to Anticoagulation at Trillium Health Partners. This full-time role combines clinical expertise, problem-solving, and collaboration with healthcare teams across various locations.
As a permanent full-time Pharmacist within the Pharmacy department, you will manage pharmacotherapy for patients and engage in evidence-based practices. Your responsibilities will include comprehensive pharmaceutical care in outpatient and inpatient settings, alongside staff training and collaboration with healthcare professionals. A proactive approach alongside strong interpersonal skills is essential.
Key Responsibilities:• Provide comprehensive pharmaceutical care to patients • Manage drug therapies in anticoagulation and other clinical programs • Collaborate with healthcare teams to optimize patient outcomes • Conduct educational activities for residents and students • Participate in pharmacy case presentations and research initiatives
Requirements:• Baccalaureate Degree in Pharmacy or Pharm D • Licensed in Ontario; residency experience preferred • Strong problem-solving and communication skills • Relevant clinical experience in hospital pharmacy • Ability to adapt in a dynamic healthcare environment
