Bayshore HealthCare is looking for passionate Social Workers to provide essential services and support to clients. You will be involved in assessing social, emotional, and cognitive needs while devising effective treatment strategies. This position requires full-time availability primarily during weekdays, with around 30 visits per week, and emphasizes a person-and-family-centered care approach.

Key Responsibilities:• Manage client and family communications collaboratively • Assess and adjust treatment plans based on client needs • Teach essential coping skills for emotional well-being • Advise on community resources for support • Coordinate with other healthcare and community professionals

Requirements:• Must be an active member of the Ontario College • Prefer two years of community therapy experience • Proficient in English; French considered beneficial • Valid driver’s license with vehicle access preferred • Provide police clearance and current CPR certification
