Position Summary

As a member of a multi‑disciplinary team, the Registered Practical Nurse practices in accordance with the relevant legislation/standards, the UHN Professional Practice Model, and the mission and vision of nursing at UHN to provide compassionate, collaborative, and competent care while meeting the legal and ethical requirements of the profession.Job Details

Site: UHN Reactivation Care Centre– 82 Buttonwood York, ON M6M 2J5 Department: Transitional Care Program Reports to: Manager, Program Services Hours: 22.5 hours per week Wage Range: $38.42 – $39.02 per hour (start rate non‑negotiable) Shifts: 0715–1915, 1915–0715 – Rotating Days/Nights/Weekends/HolidaysStatus: Permanent Part‑Time Closing Date: May 20, 2026 Duties

Plan, manage, and evaluate nursing care for patients and families within the RPN scope of practice in the context of an interdisciplinary practice. Use current theoretical and research knowledge and the input of team members to plan and implement care for patients and families. Participate in inter‑professional team care planning forums, inter‑professional rounds, and family conferences.Provide mentorship to RPN students and new staff on the unit. Participate in qualified, program, and hospital‑wide initiatives, including professional development of self. Maintain compliance with patient safety responsibilities and corresponding hospital plans, policies, and procedures to ensure a safe environment for patients.Qualifications

Valid registration with the College of Nurses of Ontario. Diploma in Practical Nursing. Recent related clinical experience preferred. Effective problem recognition, problem solving, and decision making skills. Demonstrated commitment to continuing professional development. Demonstrated commitment to team success.Resilience and effective coping strategies to manage change. Demonstrated computer literacy in word processing and Electronic Patient Record. Working knowledge of the College of Nurses of Ontario standards of practice. Evidence of satisfactory attendance and punctuality. Benefits
