May 21, 2026 About the McCall Centre McCall Centre is a 120‑bed chronic care facility situated on the Queensway Health Centre campus. It provides transitional care for patients to reintegrate into the community. Position Summary

Responsible and accountable for the provision and management of nursing care for patients. Provides nursing assessment, contributes to the care plan development, provides goal‑oriented treatment/interventions, patient and family education, and support to address medical, cognitive, emotional and self‑care needs.Contributes to the development of a realistic discharge plan. Acts as a coach/mentor/consultant and uses sound critical thinking skills key to patient outcomes and facilitating team functions. Participates in program planning, continuous quality improvement and evaluation. Key Qualifications, Skills and Abilities

Graduate of a recognized School of Nursing. Current certification or eligibility for certification of competence from the College of Nurses of Ontario as a Registered Practical Nurse. Current BCLS required. Recent experience with inpatient medical population. Recent experience with inpatient rehab population and/or complex care needs population.Recent knowledge of the philosophy and concepts of geriatric nursing practice. Demonstrates ability to provide patients and families with educational resources. Demonstrates patient and family‑centered care. Demonstrates knowledge and ability to establish a therapeutic relationship. Evidence of ongoing professional development.Must be reliable, versatile, creative, self‑directed to work effectively and collaboratively in a high‑performance inter‑professional team. Satisfactory attendance and performance required.
