Position Summary: The Assessment and Brief Services Case Manager provides a local, equitable, open-door point of assessment for individuals seeking mental health and substance use health supports at CMHA HPE. Through standardized screening, triage, and risk assessment, the role supports timely, person-centered access to care. The position delivers immediate brief services, including single-session interventions, crisis planning, and short-term, task-based supports, while facilitating warm transfers to CMHA HPE programs and appropriate community resources. The Case Manager also provides ongoing interim support for individuals awaiting services by maintaining waitlists, offering proactive check-ins, re-triage, and connection to groups, psychoeducation, and other supports that promote engagement and safety. Working collaboratively with clients, families, caregivers, primary care, hospitals, and community partners, the role contributes to coordinating care, accurate documentation, and service planning within a fast-paced environment. This role is grounded in trauma-informed, culturally safe, anti-oppressive, and recovery-oriented practices. This is a float position with Belleville as the home office location, and with travel throughout Hastings Prince Edward to provide coverage at other office locations including Picton, Centre Hastings, and Quinte West. Minimum Qualifications & Experience: - University degree or college diploma in a relevant area of study (e.g., social services, psychology, mental health, addictions, or related discipline). - Minimum 5 years of recent and relevant experience providing mental health and/or addictions services. - Equivalent combination of relevant education and experience may be considered. - Current certification in First Aid and CPR. - Current Applied Suicide Intervention Skills Training (ASIST) is an asset. - Current Non-Violent Crisis Intervention (NVCI) certification is an asset. - Ability to work under pressure within a changing environment. - Demonstrated experience conducting assessments, screening, triage, and risk assessment. - Experience delivering brief, evidence-based interventions and short-term supports. - Proven ability to support individuals while awaiting service attachment, including waitlist management and interim support provision. - Strong knowledge of community mental health and addictions systems, resources, and relevant legislation. - Demonstrated commitment to trauma-informed, culturally safe, and anti-oppressive practice. - Strong organizational and time management skills, with the ability to manage competing priorities in a fast-paced environment. - Demonstrated professionalism, sound clinical judgement, effective boundary-setting, and emotional self-regulation. Working knowledge of the Health Care Consent Act, Mental Health Act, Personal Health Information Protection Act, and other relevant legislation. - Excellent communication and interpersonal skills to effectively engage community services and individuals, increasing awareness of services. - Well-developed problem-solving, prioritization and conflict resolution skills. - Ability to work autonomously as well as collaboratively in a multidisciplinary team setting. - Basic proficiency in computer skills; MS Office preferred. - Must possess a valid Ontario Driver’s License, have regular access to a reliable vehicle, provide proof of adequate vehicle insurance, and a clear driver’s abstract. - Required to provide a satisfactory criminal record check (CRC) and Vulnerable Sector prior to hire.
