Physiotherapist, Regular Full Time, Renfrew, Labelle, Pembroke, Smith Falls– Ontario Health atHome Join to apply for the Physiotherapist role at Ontario Health atHome. This position involves assessing patient care needs, providing access and referrals to community services, and engaging with patients, caregivers and other health care practitioners. You will work in office, in the community, or in a health care facility, providing connected, accessible, patient-centered care as part of a collaborative team of health care professionals.What will you do?
The Physiotherapist plans, organizes and directs diverse programs of treatment, counseling, screening and education for Ontario Health atHome Champlain Patients in homes and retirement homes. Provide physiotherapy services through initial assessment, identification of problems, establishment of goals and treatment plans, implementation of physio treatment, follow-up and management of patient’s condition.Establish priorities and treatment goals and develop methods to achieve them. Maintain clinical records in accordance with Ontario Health atHome Champlain Policies and Procedures and the College of Physiotherapists of Ontario. Schedule patient visits according to physiotherapy priorities. Collaborate as a member of a multidisciplinary team and maintain liaison with other health care professionals.Act as an education and resource person for other health care professionals. Participate in committees as approved and engage in professional development to maintain knowledge in physiotherapy. Maintain supplies and recommend equipment for department use within budget. Interpret the role and objectives of the Ontario Health atHome Champlain Physiotherapy Program in the community.What must you have?
Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario Related professional experience normally attained over two years in a Physiotherapy role Related experience in a similar community-based setting is an asset Current CPR-C certificateCOVID-19 vaccination as a condition of employment with proof of vaccination prior to start date What would give you the edge?

Knowledge of the health care system and the role of Ontario Health atHome Champlain within the system Knowledge of Ontario Health atHome Champlain operations, quality, risk, and performance management principles Knowledge of relevant health care legislation Knowledge of Ontario Health atHome Champlain policies, procedures, strategic direction and goalsUnderstanding of Ontario Health atHome Champlain partners and contracted service providers in the community Skills And Abilities
Effective planning and organizing to manage multiple patients and prepare information reports Accurate documentation and familiarity with computer software (email, internet) and Microsoft Office Strong interpersonal and communication skills to work with diverse patient groups Ability to handle difficult conversations professionally and work in a team or independentlyCustomer service orientation and ability to engage with patients and families/caregivers Ability to deliver information in various settings and assist in presentations Valid driver’s license and access to a vehicle English and French is an asset Vulnerable Sector Check completed within the last six monthsHours of Work
Monday to Friday– 8:30am to 4:30pm (35 hours/week) What do we offer?
