Job Description In this community nursing role, you as a Registered Practical Nurse (RPN) will conduct scheduled visits within the community to care for patients of all ages in various settings. You will work alongside RNs for more complex patients and play a key role in helping patients remain comfortable in their homes for as long as possible. This position offers autonomy, independence, and a work‑life balance.

Responsibilities
Participate in scheduled visits to look after patient needs within your scope of practice
Provide ethical and safe nursing care
Use appropriate assessment tools and techniques to assess patients’ individual needs
Execute and continuously monitor care plans, adjusting as required
Engage patients collaboratively by building rapport and trust
Maintain and update professional patient records related to encounters and plans
Qualifications
Registration with the CNO as a Registered Practical Nurse in good standing
Valid driver’s license and access to a vehicle
Experience in delivering community care is an asset
Reliability and flexibility
Willingness to travel to and from patient residences within the designated area
Experience working with the vulnerable population
Passion for making a difference in the community
Required on‑call coverage, including after hours to ensure patient safety and to meet patient care needs
Compensation and Scheduling Full‑time: $26.27 – $29.11 per hour. Salary offers are based on your skills, experience, education, and alignment with the responsibilities of this position. Comprehensive benefits support your health, financial well‑being, and long‑term career growth.
