In this role, you’ll deliver comprehensive, patient-centered care and collaborate with an interdisciplinary team across prevention, treatment, chronic disease management, and health promotion.Region / Work Area : Brockville & Mallory town (some travel may be required) Candidates from Ottawa Region | Kingston are encouraged to applyCompensation & Schedule* Pay: $68.55/hour* Hours: 37.5 hours/week (weekdays + some evenings; adaptable work week)Key Responsibilities* Provide comprehensive primary care: assessment, diagnosis, treatment, follow-up, and referral* Perform advanced health assessments; order/interpret diagnostics; prescribe and manage medications within scope* Manage acute and chronic conditions; support complex patients with ongoing care plans* Deliver health promotion, disease prevention, and patient education/counselling* Collaborate with physicians and allied health professionals in an interdisciplinary setting* Maintain timely, accurate, and complete clinical documentation (EMR/clinical records)* Support care coordination, continuity of care, and patient navigation to community resources* Contribute to quality improvement and evidence-informed practiceQualifications & Experience* Nurse Practitioner (Primary Care & Adult) with current registration in good standing with the College of Nurses of Ontario (CNO)* Strong clinical judgment, critical thinking, and decision-making ability* Ability to work independently while thriving in a collaborative, interdisciplinary environment* Community-based primary care experience and/or experience supporting individuals with complex needs (asset)### Skills* Excellent verbal/written communication and strong interpersonal skills* Highly organized; accurate; robust attention to detail* Strong computer skills; confident with clinicaldocumentation standards* Demonstrated commitment to respectful, inclusive care across diverse backgrounds### Requirements* Satisfactory Vulnerable Sector Check* Valid driver’s license and $2M vehicle liability insuranceBenefits (high level)* Vacation, pension, medical/dental coverage, personal leave* Education support (days/funding), appointment time, statutory holidays* Additional paid time provisions may apply depending on site### How to ApplyTo be considered, apply directly through this LinkedIn job posting.
