Description
Location 150 - 8518 Glenlyon Parkway, Burnaby, British Columbia, V5J 0B6 Why this role is important? The Nurse Case Manager will respond to patient inquiries, maintain a patient caseload, utilize effective communication skills, provide problem‑solving abilities both in a team environment and independently. Building strong relationships internally and externally is key to meeting team and business objectives. The role also involves attending pre‑booked training sessions, completing self‑learning assignments, and adhering to SDM Specialty Health Network SOPs, Policies, and College of Nurses Standards of Practice. What you will do - Manage assigned caseload. - Act as the primary resource and contact for patients, physicians, health care professionals, and vendors within the designated area. - Coordinate and facilitate patient progress from enrollment to drug administration and throughout therapy. - Facilitate self‑care through advocacy, shared decision‑making, and education. - Practice cultural competence and respect for diversity. - Maintain thorough documentation in patient files to ensure continuity of care and transition of changes in patient care. - Provide status updates related to reimbursement, coordinate nursing requirements, blood work, pharmacy dispensing/delivery/refill requests. - Report all adverse events (AE) and product technical complaints (PTC) immediately or as soon as possible. - Promote communication and coordination among MDs, pharmaceutical partners, patients, clinic staff, and other SDM departments. - Respect confidentiality and advocate for patients/families to achieve positive outcomes, resolving ethical issues according to the Canadian Nurses Association Code of Ethics. - Complete occasional patient education visits requiring travel and overnight stays. - Participate in internal and external committees. - Plan and implement in‑services and continuing education programs for stakeholders as requested by management. - Demonstrate commitment to continuous quality improvement. - Maintain high professional standards, adhering to ethical guidelines and staying abreast of professional advances. - Engage in ongoing reflective practice activities. - Assume responsibility for personal and professional development. - Other responsibilities as required by SDM Specialty Health Network. What you will need - Five or more years of direct clinical experience, ideally in rare disease, respirology, or cardiology. - Current professional nursing license in the province of work. - Fluency in both French and English is an asset. - BScN preferred; a combination of experience and education will be considered. - Organized, efficient approach to case management. - Excellent assessment, communication, critical thinking, and decision‑making skills. - Understanding of SDM policies, legal considerations, and professional ethics. - Experience with Health Canada guidelines for reporting adverse events is an asset. - Computer proficiency: Microsoft Outlook, Word, Excel, Internet Explorer; experience with Salesforce. - Presentation skills for colleagues. - Objective, team‑player attitude. - Adaptability to a changing setting. - Commitment to quality client‑centered service delivery. - Knowledge of continuous quality improvement processes. - Knowledge of case‑management processes. - Ability to travel for the role. - Valid driver’s licence.
