Elevate resident care as a Registered Practical Nurse with S&R; Group. In this full time role, you will lead rehabilitation efforts and support frontline teams to enhance resident outcomes. As the Resident Care Coordinator and Rehab Lead at S&R; Group, your leadership directly impacts daily care practices in the home. You will utilize your clinical expertise in geriatric nursing, managing rehabilitation programs and leading a dedicated PSW team. Your proactive approach ensures that resident-directed care principles are upheld while promoting quality improvement initiatives. Key Responsibilities: • Lead rehabilitation programs for resident mobility and fall prevention • Provide clinical leadership as a Registered Practical Nurse • Oversee compliance with health and safety standards • Manage communication and training for PSW team members • Conduct care conferences for effective resident planning Requirements: • Registration in good standing with the College of Nurses of Ontario • Vulnerable Sector Screening completed within 6 months • Experience in geriatric nursing and long-term care • Strong interpersonal and organizational skills • Proficient in quality improvement activities and Microsoft Office Bring your passion for geriatric care and positive leadership to S&R; Group as a Resident Care Coordinator. #J-18808-Ljbffr Apply on Kit Job: kitjob.ca/job/2ndtq3
