Develops individual action plans and strategies to focus own performance towards the goals of the strategic plan. Formulates, monitors, and evaluates nursing practice to ensure quality professional care and standards ofclinicalpractice comply with professional, legal and licensing requirements. Maintains broad internal and external contacts at a senior level for the purpose of enhancing the delivery of care programs and services for the organization. Collaborates with physicians, medical specialists, pharmacists, foot care nurse, dental hygienist, and other allied health professionals to ensure delivery of a continuum of integrated resident Performs Home centered audits as scheduled. Participates in discharge planning activities. Monitoring, ordering and stocking of care supplies. Works under the direction of the Director of Care to ensure ClinicalKPI and care needs of residents are met Reports to the Care Manager for all human resource and employment related matters Works as unit nurse under emergent situations as needed Covers the Care Manager when on vacation to support in staff related concerns with the HR&Scheduling; Coordinator Must Have Qualifications - Are YOU the Right Fit?
