Social Service Worker at Regency Place, Port Hope

Contribute to resident well-being at Regency Place Long Term Care Home as a full-time Social Service Worker. Provide emotional support and advocate for residents in a compassionate setting.

This permanent full-time role focuses on supporting residents' psychosocial needs and ensuring their dignity and independence. You will collaborate with families, staff, and community resources to deliver essential services, including counseling and palliative care. The position requires working at both Regency Place and Hope Street homes over five days biweekly.

Key Responsibilities:• Coordinate interdisciplinary care conferences for admissions • Develop and implement social work services for residents • Provide counseling and emotional support when needed • Offer palliative care support to residents and families • Address concerns of potential financial or emotional abuse

Requirements:• Bachelor/Master of Social Work (BSW/MSW) or SSW • Registration with Ontario College of Social Workers • Experience in long-term care or healthcare • Knowledge of relevant legislation and residents’ rights • Strong communication skills with a compassionate approach
