In this role you will be the go-to-person ensuring patient records are accurate, complete and ready when clinicians need them. The HIM Clerk works hands-on with health information systems and the opportunity to learn new tools that are shaping the future of healthcare data. We offer 14% in lieu of benefits - At this time this opportunity is scheduled 8-hour days shifts Monday to Saturday As the successful applicant, you will do the following: - Scanning of all inpatients, day surgery, emergency and outpatient visits including mental health reports - Responds to requests from internal/external sources for information regarding confidentiality and release of information policies and procedures - Fulfillment and management of Release of Information (ROI) requests (Legal/Insurance, Circle of Care, Patient) - Prepare charts for scanning, scan documents, perform QA, index documents in accordance with established procedures for all patient types i.e. inpatient, day surgery, ER, outpatient, Mental Health etc. - Performs audits and other data quality activities related to patient charts - Review and update dictated reports through the DeliverHealth/DMO transcription module including maintenance of DMO physician database - Work with Health Coding team to fulfill NSQIP program - Maintain log of path specimens and provide monthly statistics - Archive old charts including destruction process - Review of reports for administrative quality and review of health information ACRs - Communication with Care4 regional partners on administrative changes - Quarterly regional e-scription checks - Physician dictation administrative duties (access, e-scription, amendments) - Mail sorting and distributing in mailboxes - Ensuring data privacy and protection of health information, including logging breaches with Regional Privacy team - Ordering supplies for departmental needs - Scanning NST strips What You’ll Bring Required - OSSD or equivalent - Medical Terminology course or working knowledge gained through recent and relevant experience - Experience with Release of Information (ROI) for patients, legal, insurance, and Circle of Care Preferred - Knowledge of applicable legislation including Public Hospitals Act, Mental Health Act, PHIPA etc. - Familiarity with solutions such as PocketHealth, Active Fax, Net Delivery, Outlook mailboxes and other technologies related to ensuring patient information is secured, curated, and stored safely and timely within patient’s chart - Familiarization with Meditech Expanse Alignment with our Values We Are Committed To Our Values Be Kind - fostering a culture of compassion, empathy, and dignity in every interaction Support Each Other - working together across all roles to uplift our team and our patients Lead by Example - setting a standard of integrity, fairness, and professionalism Keep Growing - embracing curiosity, innovation, and learning to adapt to future needs Any successful candidate will need to live these values on a day to day basis.
