Bayside Health was formed following the merger of Alfred Health, Bass Coast Health, Gippsland Southern Health Service, Kooweerup Regional Health Service and Peninsula Health on 1 January 2026. - Part time (64 hours per fortnight) - Social Worker Grade 3, Year 1‑4 (SC31‑SC34) - Located at Sandringham Hospital The Department The Social Work Department operates in accordance with the philosophy and goals of The Alfred. A focus on quality of life and participation in care is a primary goal of the Department. Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care. Social Work also assists patients and their families with a smooth transition from hospital to home. Social Work Provides a Range Of Services Including - Counselling - Discharge Planning - Crisis interventions - Bereavement counselling - Family support - Care coordination - Making Appropriate referrals - Community liaison The Role A fantastic prospect exists for an experienced Social worker to work at Sandringham Hospital as the Team Leader. You will have demonstrated prior experience in managing a complex clinical caseload and will have sound knowledge and skills in hospital discharge planning and providing complex interventions to facilitate discharge. You will need to have relevant leadership experience in a hospital setting and experience in managing a small team. An understanding of grief and loss counselling, NIDS, VCAT and psychosocial support is also essential. You will possess strong assessment, case planning and have experience in providing staff and student support. You will work within a strong multidisciplinary team and will be part of an organisation where professionalism, innovation and the ability to find creative solutions to challenges is highly regarded. You will be required to manage daily workload allocation, leave planning and provide operational support and supervision to a small team. This role will require the applicant to provide leadership to the clinical units within Sandringham Hospital. The successful applicant will need to contribute to quality improvement and research activities and provide staff and student supervision. Occasional weekend availability is desirable for this role. Qualifications And Experience Required - Applicants must hold a Bachelor Degree in Social Work - Applicants must have an understanding of the psychosocial issues associated with this patient cohort - Substantial experience in working within a hospital setting will be essential to this role Essential It is vital that the successful candidate has experience in working within an Electronic medical record. A knowledge of NDIS, Aged care and VPTAS processes is essential for this role. Please ensure you address the Key Selection Criteria in your application.
