Salary: CAD $54.49 - $68.10/hour
Job Summary Full‑time Clinical Pharmacist at Delta Hospital located in Delta, BC, responsible for providing individualized pharmacotherapy, patient counselling, and collaborative medication therapy management.
Responsibilities
Consult with physicians, nurses, and other health care providers to develop and discuss therapeutic plans and ensure appropriate documentation.
Individualize patient medication therapy by reviewing literature, patient charts, lab results, and assessing clinical response.
Provide patient medication counselling and educational support regarding drug therapy.
Assist in developing drug therapy guidelines and contributing case studies and review articles to professional journals.
Participate in ward rounds, program team meetings, and pharmacy services at the unit level.
Serve as a department resource person, providing detailed drug information.
Provide instruction and supervision to pharmacy students and residents.
Participate in quality improvement, medication safety initiatives and clinical drug trials.
Perform drug distribution duties, including interpreting prescriptions, dispensing medications, and maintaining accurate records.
Review prescriptions for therapeutic suitability and communicate with prescribers for clarification.
Maintain records for narcotic and controlled drugs, special access programs, and departmental statistics.
Prepare reports on a variety of activities and maintain inventories, requisitions, and expiry monitoring.
Perform other related duties as assigned.
Qualifications
Current registration as a Pharmacist with the College of Pharmacists of British Columbia.
Completion of an accredited Hospital Pharmacy Residency Program or equivalent combination of education and experience.
Skills and Abilities
Broad knowledge of organization policies, procedures, and standards of care.
Knowledge of evidence‑based clinical pharmacy practice and medication use evaluation.
Knowledge of research processes and drug distribution in a hospital setting.
Ability to apply clinical pharmacy knowledge independently.
Effective verbal and written communication skills.
Ability to establish and maintain effective working relationships.
Ability to organize and prioritize work and operate related equipment.
Physical ability to carry out the duties of the position.
