Care Manager for Assisted Living Community, Dollard-des-Ormeaux

As a Care Manager, you will be responsible for closely monitoring residents' health while adhering to their Individualized Service Plans (ISP). This position requires a proactive approach to ensure safety and compliance while engaging residents in daily activities that promote their independence and dignity. Establish warm relationships with residents, families, and team members to create a positive community atmosphere.

Key Responsibilities:• Review resident logs for health updates • Assist with activities of daily living and social engagement • Document any changes in residents’ conditions • Support safety and risk management protocols • Foster a sense of belonging through various activities

Requirements:• High School diploma or GED as minimum • CPR and First Aid knowledge required • Experience with seniors recommended • At least 18 years of age • Strong interpersonal communication abilities
