Description
Role Summary We are looking for a dedicated and compassionate Programme Executive to support the Case Management team of our residents in our home. The ideal candidate should be detail-oriented, proactive, and committed to upholding the dignity and welfare of our residents.

Responsibilities

Review the individual care plan with relevant stakeholders

Participate in case management discussions with MSF

Work with the Case Manager on developing a system to reward or deny residents privileges to modify their behavior positively and jointly administer the program

Review all admissions, discharges, escapes from lawful custody, and deaths

Update residents’ addresses on NRIC and follow up with the replacement of NRIC when required

Source suitable jobs for able-bodied residents who can work outside the Home, and support them to adjust to the workplace and colleagues

Be involved in any court and/or police case involving the residents when the need arises

Assist in duties such as accompanying residents to the bank or prospective employer to establish a close rapport and helping relationship with them

Develop and implement programmes that enhance the residents’ quality of life within the Home

Establish indicators for residents’ performance and evaluate progress that leads to successful outcomes under the Home Earning Scheme (HES) to facilitate structured engagement and productivity

Ensure compliance with the Destitute Persons Act, Destitute Person (Welfare Home) Rules, and Vulnerable Adults Act

Document and report all major incidents (e.g., escapes, deaths, outbreaks) to the Head of Centre (HoC) and Ministry of Social and Family Development (MSF)

Participate in case conferences for residents exhibiting challenging behavior and contribute to their Individual Care Plans (ICP)

Maintain updated records on admissions, discharges, hospital transfers, escapes, and returnees

Stakeholder&Community Engagement

Maintain documentation for audits and participate in regular audits ensuring compliance with audit requirements by MSF

Prepare data for submission to MSF

Collaborate with partners to ensure clear communication of programme processes

Ensure proper application process to assess clients’ suitability for programme

Maintain regular communication with volunteers and partnering agencies to ensure clear communication of programme processes

Proficient with creating visually engaging reports and presentations

Prepare, implement and review SOPs periodically

Perform any other admin duties as assigned by the Centre Manager

Job Requirements

Degree or Diploma in any discipline; social work related or community engagement will be an advantage

Candidates with no relevant experience may apply

Strong interpersonal and communication skills; able to work independently and as part of a team

Proficiency in Microsoft Office and cloud-based systems

Incumbent may be required to work after office hours when needed
