Unit Summary Operating within the Emergency, Medicine and Critical Care Program at the Juravinski, B3 critical care unit is a specialized unit caring for Cardiac, Medical, Surgical, Orthopedic, Hematology and Oncology patients. Our multidisciplinary and cooperative team approach is dedicated to the principles of patient and family-centred care and provides this support to patients in its 6 Cardiac Care beds and 9 Critical Care beds. Position Summary The Social Worker will conduct psychosocial assessments and develop treatment plans that include counseling, education and patient and family interventions. The Social Worker will work collaboratively with hospital team members, primary care, home care and other community providers to support patient centered care including seamless and timely transitions between hospital and community. Organizing and leading meetings with the multidisciplinary team to develop holistic care plans for patients with complex health and social needs will be a key responsibility of the Social Worker Schedule Work Hours Monday to Friday, days Qualifications 1. Master''s Degree in Clinical Social Work from an accredited University 2. Current registration and member in good standing with the Ontario College of Social Workers and Social Service Workers 3. Minimum 3 years recent experience with adults and families providing individual and family therapy 4. Minimum of 3 years of critical care experience preferred 5. Previous experience in providing bereavement crisis and counseling 6. Knowledge of Healthcare Consent Act 7. Knowledge of community resources and referral process 8. Demonstrated skills in the performance of a therapeutic role including strong assessment, counseling, case management, crisis intervention and advocacy skills 9. Proven ability to work in a fast-paced, complex environment and to organize, prioritize and problem-solve effectively 10. Strong interpersonal, communication, teamwork, consultation and teaching skills 11. Demonstrated ability to contribute to an Inter-professional model of care team utilizing Family centered care principles 12. Demonstrated commitment to ongoing learning 13.
