Summary As a Social Worker at Age-Wise Solutions Inc., you will play a vital role in supporting the emotional and psychological well-being of residents and their families. This position offers the opportunity to work with a diverse caseload, addressing issues such as loss and grief, mental health, and family dynamics. Responsibilities Provide psychosocial support to residents in long-term care and retirement homes. Conduct assessments to identify the needs of residents and develop appropriate intervention plans. Collaborate with families and caregivers to address concerns related to mental health and family dynamics. Advocate for residents’ rights and contribute to improved social conditions. Participate in regular peer-to-peer support meetings with fellow professionals. Engage in continuous professional development opportunities to enhance skills. Requirements Bachelor's or Master's degree in Social Work from an accredited institution. Registered member of the Ontario College of Social Workers and Social Service Workers. Experience in case management is essential; experience with elderly populations is an asset. Strong communication skills, both verbal and written, for effective interaction with residents, families, and staff. Ability to work independently as well as collaboratively within a team workplace.
