About the Clinic We are a community-based primary care clinic providing comprehensive, patient-centred care across the lifespan. Our team includes family physician ,allied health professionals, and administrative staff. We focus on timely access, prevention and chronic disease management, and a welcoming experience for every patient. Position Summary As a Registered Practical Nurse (RPN) in our primary care clinic, you will support physicians and the interprofessional team by providing nursing care, performing clinical procedures, contributing to preventive care and chronic disease management, and coordinating follow-up to help patients navigate their care. The ideal candidate is organized, calm under pressure, and committed to respectful, culturally safe, trauma-informed care. Key Responsibilities - Perform nursing assessments (e.g., vitals, focused assessments) and document findings accurately in the EMR. - Provide in-clinic support for scheduled and walk-in visits as appropriate (rooming patients, medication reconciliation, history updates). - Administer injections/immunizations, - Assist with minor procedures (e.g., wound care, dressings, suture/staple removal, ear lavage) and maintain procedure trays/supplies. - Support chronic disease management and preventive care (e.g., diabetes, hypertension, COPD/asthma education; screening reminders and follow-up). - Triage patient calls/portal messages within scope, using established protocols; escalate concerns to the appropriate clinician. - Coordinate referrals, follow-ups, and patient education; - Maintain vaccine cold chain, infection prevention and control (IPAC) practices, and clinic safety standards. - Contribute to quality improvement initiatives - Support a respectful environment by collaborating with colleagues and communicating professionally with patients and families. Required Qualifications - Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Practical Nurse (RPN). - Current CPR (BLS) certification. - Demonstrated competence in primary care nursing skills (e.g., immunizations, injections, wound care, point-of-care testing) or willingness to be trained. - Experience using an electronic medical record (EMR) and strong documentation practices. - Knowledge of infection prevention and control (IPAC) standards and routine practices. - Strong interpersonal communication, time management, and ability to prioritize in a busy clinic setting. - Ability to work independently and collaboratively within an interprofessional team. - Clear vulnerable sector check (or ability to obtain, as required by clinic policy). Preferred Assets - Experience will be an asset but not required. - Experience with immunization programs (including cold chain monitoring) and/or travel vaccines (if applicable). - Experience supporting chronic disease programs (e.g., diabetes education, spirometry support, anticoagulation support—if within clinic services). - Familiarity with Ontario health system navigation, community resources, and referral pathways. - Additional certification/training (e.g., foot care, smoking cessation, wound care, phlebotomy). - Second language skills considered an asset: [list if applicable]. Schedule & Work Environment - Hours: Monday, Tuesday and Thursday from 9:00 AM to 5 PM.(24 h weekly) - Start date: ASAP - This role is in-clinic and involves standing, bending, and assisting with clinical procedures throughout the day.
