Responsibilities
Communicate with clients, families/caregivers, service providers and other professionals to collaborate and coordinate services Maintain and manage a caseload in an efficient and effective manner ensuring priority code timeframes are met Assess the client’s adjustments to medical, emotional, psychological or social problems and the factors that affect themUtilize a client-centered approach to identify, prioritize and monitor potential psychosocial problems and goals Establish, implement, evaluate and adjust the psychosocial treatment plan in collaboration with the client and or family, caregivers, care coordinator and other community partners Enhance or teach coping skills e.g. relaxation, stress management, anger management, healthy grievingAdvise clients/caregivers on the availability of community resources and contact agencies on their behalf when appropriate e.g. Federal, Provincial, Municipal and Local agencies in relation to income security, pensions, financial assistance, etc. Determine when further social work intervention is no longer necessary and assist clients in planning for dischargeRecommend and facilitate referrals to support services within the community Comply with continuous quality improvement initiatives of Bayshore Develop a flexible, responsive system to meet changing and competing demands Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating bodyLiaise with Case Managers, funders or Government&hospital programs as required Liaise with Provincial regulating body as required Adhere to Bayshore Policies and Procedures Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System Participate in proactive Health&Safety activities while performing all duties, and report any Health&Safety risks or concerns to immediate Supervisor Actively participate in and demonstrate support for continuous quality improvement initiatives Maintain positive work and attendance performance Maintain confidentiality of client and corporate information and discuss it only with appropriate Bayshore personnelComplete other tasks as requested Education
Possess a Master's and/or Bachelors Degree in Social Work Qualifications and Requirements
Be an active member in good standing of the Ontario College of Social Workers and Social Service Workers Retain professional liability insurance ($5 million minimum) Provide a police clearance that meets the Bayshore Home Health policy Possess general computer skills and have access to a computer with appropriate password protection and internet capabilitiesMinimum of two years of community-based therapy experience (preferred) Be able to drive, hold a current, valid driver’s license and have access to a vehicle in good operating condition (preferred) Must carry two million dollars in automobile insurance coverage if utilizing personal vehicle for appointmentsHold a current CPR certificate (Basic Rescuer) Additional Requirements
Dependent upon Provincial government regulations, and the position being applied for, new hires may be required to provide proof of vaccination against illnesses such as COVID-19, Hepatitis, Seasonal Flu etc. Medical exemptions or any other kinds of requested exemptions based upon Human Rights Code will be considered on a case‑by‑case basis.EEO Statement
