Qualifications - Registered and in good standing with the College of Dietitians of Ontario - Must provide proof of liability insurance - Must provide proof of TB testing - Must provide proof of a vulnerable sector check - Ability to work both independently and within a multidisciplinary team - Strong written and verbal communication skills - Excellent organizational and time‑management abilities - Demonstrated problem‑solving skills - Proficiency with computers, including experience using electronic medical records (EMR) - Travel may be required for certain positions Responsibilities - Identify and monitor the level of nutrition and hydration risk for all residents - Complete scheduled RAI assessments on admission, quarterly and for significant status changes - Prioritize and respond to dietary referrals in a timely manner - Develop, evaluate and modify individualized nutrition care plans - Liaise with residents, families/SDMs, and the team to implement effective dietary interventions - Conduct swallowing assessments as required - Provide nutrition counselling and education to residents and their families - Collaborate with the Nutrition Manager to enhance the dining experience - Participate in care conferences and committee meetings as per organizational needs - Review and approve seasonal menus and create individualized menus where applicable - Provide training and support to the dietary and nursing team - Contribute to continuous quality improvement (CQI) initiatives - Ensure all work complies with Ontario Regulation 246/22, Section 80, under the Fixing Long‑Term Care Act, 2021 - Perform additional duties as required We thank all applicants for their interest.
