Key Responsibilities:• Utilize assessment tools to evaluate patient needs • Continuously monitor and update care plans • Engage patients collaboratively to build trust • Ensure accurate maintenance of patient records • Travel within a designated area for home visits

Requirements:• Registration with CNO as a Registered Practical Nurse • Mandatory experience in Pediatric care • Valid driver's license and vehicle access • Clinical experience in gastric feeding preferred • Willingness to provide on-call coverage
