Description
We are looking for a Nurse Specialized in Wound, Ostomy & Continence (NSWOC) . The NSWOC will provide wound and ostomy consultation for patients in the clinic and community. If you enjoy caring for a broad range of clients with a variety of wound etiologies and wound care modalities this is the job for you. In addition to make full use of your wound and ostomy care knowledge, each day you will use your skills in promoting self-management by patients and leading the interdisciplinary team to achieve optimal patient outcomes. Your expertise in the management of wounds and ostomies will be sought out by individual practitioners and the various committees and organizations that guide wound care in the region. CarePartners will provide you with an in-depth orientation to both the agency and wound/ostomy care in the community; ongoing clinical support including online access to clinical expertise; We are looking for Nurses (RN) with NSWOC certification or IIWCC certification who desire great flexibility in their schedules. General or non-NSWOC related visits are compensated at our RN rates. Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience. Using your knowledge of assessment, prevention and interventions regarding wound infections and ostomy care Care for patients with consuming wounds, ostomy, continence, provide lower leg assessment including ABPIs critically appraising the wound care literature and apply research evidence to practice Monitoring criteria for adjunctive therapy for complex or recalcitrant wounds Supporting our patient’s safety by practicing according to policy and standards Using your professional assessment skills to perform a thorough holistic assessment of each client and identify the client's needs by utilizing information from the client, caregiver(s), the home environment, the Ontario Health atHome referral and other sources Functioning as a member of a nursing team who are accountable for the nursing care it provides Developing appropriate time-specific, measurable goals and outcomes in collaboration with the client and/or caregiver(s), and in keeping with the Ontario Health atHome goal for treatment Registered Nurse (RN) diploma Current College of Nurses of Ontario (CNO) registration in positive standing Graduate of Nurses Specialized in Wound, Ostomy & Continence Canada (NSWOCC, formerly CAET), CNA exam may be pending A valid driver’s license and vehicle as regular travel is required Excellent English verbal and written communication skills Current immunization record including 2 step TB test CarePartners In Your Community In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings.
