Program Overview Amplify Care is a not-for-profit organization (founded in 2014 as the eHealth Centre of Excellence) dedicated to helping clinicians and their teams thrive through the implementation of innovative digital health and AI solutions. We focus on technologies that truly meet clinical needs: streamlining workflows to reduce administrative burden, supporting evidence-based decision‐making at the point‐of‐care, enhancing clinical pathways, improving interoperability between disparate information systems, and advancing data quality to support proactive care. Position Description Reporting to the Director of Integrated Care, the Manager of Integrated Care will lead and support the Director of Integrated Care with executing engagement strategies with key partners to explore innovative solutions and continuously improve clinical workflows. This position is for a creative leader with a passion for advancing patient care and digital health; who is also an exceptional person skilled at inspiring and engaging with others. It is a position for those who recognize the need to get the right thing done and pitch in to accomplish important goals. The Manager of Integrated Care will work as part of the Integrated Care team to act as a central support and subject matter expert for key collaborators such as Ontario Health Teams (OHTs) to tackle ongoing challenges within healthcare. Key Roles and Responsibilities Lead and support efforts across the Integrated Care team to liaise with key collaborators (e.g., OHTs, PCNs) and represent Amplify Care as trusted partners and experts in integrated care Collaborate and build consensus with external partners to support the development of virtually enabled models of care Convert partner‐provided direction and information into strategically sound ideas and successfully implemented deliverables Drive innovation by continuously taking initiative to identify improvements in clinical workflows Work with the Director of Integrated Care to execute activities and strategies in alignment with Amplify Care's mission to be the leading, trusted, digital health partner for primary care and integrated patient care Anticipate, understand, and respond to the needs of internal and external partners to meet or exceed their expectations within organizational parameters Support the development of efficient and suitable strategies to deliver integrated care initiatives Collaborate with Integrated Care team members to share knowledge and mine insights from emerging trends and prospective new integrated care solutions Regularly report updates to the Director of Integrated Care Perform other duties as required Experience, Skills and Qualifications Minimum 5 years of relevant experience, including leadership of cross‐functional and multidisciplinary teams and oversight of information systems and projects in healthcare Post‐secondary degree in Health Informatics, Information Technology, Business Administration, Leadership or equivalent combination of relevant education and experience Change management experience and certification an asset Project Management Professional certification or equivalent an asset Proven track record of success in establishing partnerships with healthcare organizations Knowledge of emerging technology trends in healthcare Knowledge of health information systems, technology, and software Strong conflict resolution, facilitation, negotiation and mediation skills with the ability to build consensus across a diverse group of collaborators Exceptional communication skills with the ability to effectively engage, collaborate, and build partnerships with key internal and external collaborators Demonstrated agility in learning and acclimating to unfamiliar business areas or technologies Prior experience working efficiently in a diverse and fluid working environment Ability to travel within Ontario as required Employment Equity Statement We encourage applications from all qualified candidates including 2SLGBTQIA+ identified persons, persons with disabilities, First Nations, Inuit and Métis individuals, and members of Black and other racialized communities, and individuals who speak languages other than English. #J-18808-Ljbffr Apply on Kit Job: kitjob.ca/job/2psqga
