At Plan A Sudbury, we’re seeking an energetic, experienced and compassionate Nurse Practitioner to join our team to help provide outstanding care. As a Nurse Practitioner, you will practise a holistic model of care by collaborating with the interdisciplinary team, including the Medical Director. You will be contributing to the clinical goals of the home and driving resident care in a culture of excellence, innovation and collaboration. What you will bring: - Remain accountable to the Nurse Practitioner practice standard in addition to the other practice standards and guidelines all nurses must follow. - Accountable to the Medical Director and Director of Care of the Home for meeting the Long-Term Care or Retirement Home’s policies and procedures and protocols for all medical services. - Adhere to the regulatory bodies such as the Fixing Long-Term Care Act, 2021 [or Retirement Homes Act, 2010]. - Maintain confidentiality and act in an ethical manner consistent with professional expectations and the home’s code of conduct. - Collaborate with the resident, family, and health team in the development, implementation, and evaluation of the resident’s plan of care. - Participate in creating an organizational environment that supports the safety and quality of resident care and life, collaborative practice and professional growth. - Champion quality & safety by contributing to quality improvement initiatives, promoting evidence-based practices, and managing risk with a proactive approach. What We’re Looking For: - Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class (Primary Health Care or Adult) and entitled to practice with no restrictions and in good standing. - Nurse Practitioner Certification from a recognized program - Membership with the Nurse Practitioners’ Association of Ontario (NPAO) - National certification in Gerontology an asset - Bilingualism is an asset. - Minimum 1 year of experience in Long-Term Care and/or Retirement Care sector. - Provide and maintain a current CPR Certificate for the duration of employment. - Knowledgeable regarding cultural competency in professional nursing practice. - Excellent communication and people skills. - Working knowledge of Point Click Care is an asset. - Knowledge of RAI/MDS, and BSO is an asset. - Provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one. - Proof of immunizations according to Plan A Policy. - Tuberculosis testing within the last two years; Steps 1 & 2; or an X-Ray from the previous five years showing a clear result.
