Geriatric Dietitian with Interdisciplinary Team Focus, Ahuntsic North

Shape residents' nutritional health as a Geriatric Dietitian in a full-time, permanent role. Collaborate with interdisciplinary teams to develop and implement effective nutrition interventions.

This position requires conducting detailed nutritional assessments and crafting individualized care plans that address the changing needs of residents. Regular communication with various team members, including Food Service Managers, will enhance program delivery and quality care. Additionally, you will engage in education and promote understanding of dietary needs among residents and staff.

Key Responsibilities:• Conduct nutritional assessments using RAI MDS 2.0 • Develop and evaluate personalized nutrition care plans • Approve seasonal menus for nutritional quality • Engage in interdisciplinary committees on resident care • Provide nutrition education to residents and families

Requirements:• Registered Dietitian eligible for College of Dietitians of Ontario • 3-5 years’ experience in long-term care setting preferred • Familiarity with Microsoft Office and RAI MDS 2.0 • Strong organizational and multi-tasking abilities • Excellent verbal and written communication skills
