Key Responsibilities:• Identify and assess nutrition risk for Residents • Complete necessary RAI assessments regularly • Develop and implement tailored nutrition care plans • Collaborate effectively with families and multidisciplinary teams • Lead training efforts for dietary and nursing staff
Requirements:• Registration in good standing with the College of Dietitians • Strong communication and organizational expertise • Proficiency with electronic medical records (EMR) • Willingness to travel for specific roles • Must provide liability insurance and TB clearance
