Description
Wetaskiwin Home Care is seeking an experienced Registered Nurse for its Home Care team. Reporting to the Care Manager, Wetaskiwin Home Care in the role of Case Manager, you will be responsible for coordinating and providing quality care to clients in the community. Using your strong assessment, organizational, and leadership skills you will identify the needs of the client and facilitate with the interdisciplinary team the appropriate internal and external resources required. You will interact with the team and client/family to coordinate care planning. Through the development of the care plan you will ensure services are provided and modified on an ongoing basis or as needed. The successful candidate will play a key role in supporting the team in timely and appropriate referrals to stakeholders as relevant and/or provide direct care as needed. You may be required to cover other assignments as workload dictates and there is availability to take some on call shifts. As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable. Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1-4): NextGenU Health Assessment Resources Certification: Possession of a valid driver’s license and access to a motor vehicle in valuable repair for work-related travel In addition, the incumbent will be expected to demonstrate proficiency in the following areas: Actively integrates Patient and Family Centered Care principles into practice Excellent communication and interpersonal skills in order to work collaboratively with members of the multidisciplinary team Experience with Connect Care, Microsoft Office and general computer skills Minimum one (1) years community nursing experience InterRAI-HC training, Familiar with Placement process, Service authorizations, Palliative Care.
