Clinical Consultant Pharmacist - Grey County and Surrounding Area - 0.8 FTE - 6-month Contract CareRx is Canada’s leading provider of specialty pharmacy services to seniors. Position Summary
The
Clinical Consultant Pharmacist
is a member of the multidisciplinary healthcare team supporting appropriate medication use for Residents in Long-term Care Homes and Retirement Communities. The Consultant Pharmacist works with the healthcare team to review medication use to identify medication-related problems and to achieve desired Resident outcomes. The Pharmacist will provide drug information, be involved in educational activities for healthcare staff and Residents, regularly attend medication management meetings and promote Resident safety.
This position requires regular travel throughout the Grey County and surrounding area, Chatsworth, Thornbury, Owen Sound, Tiverton, Hanover.
Role Accountabilities:
Act as aliaisonbetweenlong term care homesand retirement communitiesand CareRx Pharmacyto ensure optimal Resident care, Pharmacy Services, and Clinical support
Travel to LTC homes and retirement communities
Build and maintain relationships within the community to ensure ongoing collaboration and communication between home/communityleadership teams and pharmacy teams to develop/maintain strong partnerships
Complete medication reviews in currently serviced retirementcommunities and long-term care homesserviced by CareRx Pharmacy
Complete all required documentation and billing processes as required by MOHLTC, Ontario College of Pharmacists (OCP), and other federal and provincial standards and regulations
Provide education to Residents and staff on various health-related topics including theMedsCheckprogram and attend community-based wellness fairsand other eventsas required
Conducteducation sessions withinLong Term Care Homes and Retirement Communities withinscope of practice, as applicable (e.g. policy and procedure, web portal,eMAR, influenza protocol, etc.)
Support CareRx Pharmacy policies and procedures within retirementcommunities and long-term care homes
Completemedication audits on-site in LTC Homes and Retirement Communitiesas required
Ensure regular on-site visits are made to all assigned clients
Attend/participate in annual professional conferences and trade shows, when applicable
Participate in the development and implementation of industry-leading clinical pharmacy service, when applicable
Be an education, policy and pharmacy initiative resource
Be available for special clinical team projects as required
Complete/collaborate in the develop of medication audit follow-up improvement plans when applicableWorks collaboratively with other team members to support applicable homes and communities
Participate in pharmacy-related transition processes (e.gnew client transitions,eMARtransitions, etc.)
Help with pharmacy operations/drug distribution when required
Attend and actively participate in clinical team meetings
What You Will Bring to The Team:
Insured and currently licensed in good standing with the Ontario College of Pharmacists, College of Pharmacists of British Columbia (RPhT), Alberta College of Pharmacy (ACP), Ontario College of Pharmacists
Proven ability to foster strong partnerships with other members of the healthcare team
Effective organization, planning, and time management skills
Knowledge of Kroll Windows applicationand Microsoft Office programs
Completeunderstanding ofprovincial regulations governing LTC homes and Retirement Communities
Must be familiar with the provincial standards of practice established for LTC pharmacies
Excellent customer service and interpersonal skills, oral and written communication skillsand be able to interact with persons of varying levels of professional expertise
Skilled in critical thought processes and be able to prioritize appropriately
Must demonstrate initiative and creativity
Self-directed requiring minimal supervision and able to work independently
Motivated to achieve results that reflect a high standard of quality
Good listening skills and be adaptable to changes in job environment
Dependable and responsible in the execution of job duties
Adaptable to changes within internal work structures
Strong presentation and teaching skills
Must possess a valid driver’s license
