Patient care Collaborate with key team members to actively re-assess admitted inpatients in the ED to actively plan for the most appropriate care and disposition of the patients (, admission, discharge, telemetry, etc.) to support efficient and timely patient admissions and discharges and, resolution of patient flow issues that arise including, but not limited to early discharge, transfers, diagnostic imaging, allied health assessments and interventions, MRP re-assessment and follow-up. 48_Performance Created with Sketch. Performance They will educate patients and families about unit-based processes, support admission assessments and expedite admission and discharge process for patients on the medical ward. Requirements 48_Success Created with Sketch. Qualifications Current Certificate of Registration with the College of Nurses. Current BCLS certification required and ACLS certification an asset. Minimum 3 years of recent acute medicine department experience. 1-2 years'' Leadership Experience (projects, charge nurse/team leader/supervisor/flow leader) preferred. RNAO membership is preferred.
