As a Social Worker, your primary focus will be to support the psychosocial health of seniors at Cerwydden Care and Oyster Harbour Care Centre. You will work hand-in-hand with families and clinical teams to ensure continuity of care and advocate for residents’ rights. If you thrive in a mission-driven setting, this is the role for you.

Key Responsibilities:• Conduct psychosocial assessments and provide emotional support • Guide residents through health transitions and end-of-life processes • Collaborate on care planning and multidisciplinary meetings • Educate about community resources and financial assistance • Advocate for dignity andquality of life for each resident

Requirements:• Degree in Social Work (BSW or MSW) • Active registration with BC College of Social Workers • Experience in long-term care or related field preferred • Strong relationship-building and communicative abilities • Valid driver’s license for local travel between sites
