As a Community Inclusion Worker, you will play a vital role in enabling people to achieve their personal and social goals. You'll collaborate with various stakeholders to encourage independence and self-determination. This position is key in ensuring that individuals enjoy fulfilling lives with the right support tailored to their unique needs.
Key Responsibilities:• Advocate for supported individuals’ rights and preferences • Facilitate involvement in social and vocational opportunities • Build rapport with families and community professionals • Assist in creating and implementing Individual Service Plans • Document daily activities and healthdata appropriately
Requirements:• Grade 12 education and relevant diploma • Minimum 2 years of experience in social services • Valid current first aid and CPR certifications • Proficient communication skills with families and team members • Valid BC driver’s license and clear driver’s record
