Become a part of Park Place Seniors Living as a full-time Social Worker, playing an essential role in enhancing the well-being of residents across two Vancouver Island care centres.
This position offers a blend of psychological support, advocacy, and collaborative care planning, with two days at Cerwydden Care and three days at Oyster Harbour. You will work closely with families and interdisciplinary teams, ensuring residents receive the dignity and support they deserve during transitions and health changes. Your expertise in community resources will guide families through challenges.
Key Responsibilities:• Deliver psychosocial evaluations and counseling to residents • Aid residents during critical health transitions • Facilitate care conferences and collaborative planning • Provide resources and guidance on financial matters • Advocate for quality of life and residents' rights
Requirements:• Degree in Social Work (BSW or MSW) • Registration or eligibility with the BC College of Social Workers • Preferred experience in seniors or long-term care • Strong relationship-building and communication skills • Valid driver’s license for site travel
