Community Occupational Therapist Position (Central Ontario)

Become an Occupational Therapist with The Care Company, focusing on community home care and client-centered approaches. This role involves flexibility and direct engagement with clients at home. We are seeking a committed Occupational Therapist skilled in home care to join our team. This position requires in-person visits to assist clients with chronic conditions and Alternate Level of Care. You'll develop personalized treatment plans and work collaboratively with other healthcare providers. Key Responsibilities: - Perform initial assessments and functional evaluations - Offer follow-up sessions emphasizing safety and skill progress - Develop client-centered care aligned with HCCSS expectations - Coach clients on equipment use and home safety - Document all client interactions and treatment progress clearly Requirements: - Experience with HCCSS and community-based populations preferred - Positive standing registration with COTO - Skills in electronic documentation and risk evaluation - Able to manage scheduling independently - Valid driver's license and reliable transportation Drive positive change in community health as an expert in occupational therapy.
