You will evaluate dietary needs and implement personalized nutrition plans for seniors in a compassionate setting. Key Responsibilities: • Identify nutrition and hydration risks for residents • Conduct RAI assessments regularly • Develop and modify nutrition care plans • Liaise with families and multidisciplinary teams • Provide nutrition education and counseling for residents Requirements: • Registered with the College of Dietitians of Ontario • Proof of liability insurance and TB testing • Solid written and verbal communication skills • Ability to work independently and as part of a team • Proficiency in electronic medical records Make a difference in the lives of seniors by supporting their nutrition at Seasons Care.
