Excel as a Senior Health Educator focused on community development and Advance Care Planning initiatives. This role involves delivering vital education and resources to healthcare teams and the public.
In partnership with the Director of Clinical Standards&Professional Practice, you will lead the Advance Care Planning program and develop impactful educational events. Your skills in adult education, end-of-life care, and health promotion will be critical in facilitating successful workshops and discussions that support informed healthcare choices.
Key Responsibilities:• Spearhead the Advance Care Planning educational initiatives • Formulate resource materials tailored for diverse audiences • Organize and present educational events for healthcare professionals • Collaborate effectively with a multidisciplinary team • Engage in meaningful Goals of Care conversations with clients
Requirements:• Over 5 years of experience in relevant health education or promotion • Bachelor's degree in Health Administration, Social Work, or equivalent • Strong leadership and communication abilities • Proficient in word processing and presentation tools • Registered or eligible for relevant licensing
