Job Description

The incumbent is an integral part of the Advance Care Planning Program reporting to the Director, Clinical Standards&Professional Practice (Urban). The primary focus is to implement and coordinate the Advance Care Planning initiative and to research, develop, and facilitate educational events including the development and dissemination of resource materials regarding Advance Care Planning. Education will be provided to members of the health care team across the continuum, contacts in surrounding health regions, and the general public. The incumbent is accountable to the Director of Clinical Standards&Professional Practice and works collaboratively with the Advance Care Planning Steering Committee. Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement. Experience

Minimum 5 years experience with end of life care&adult education or community development/health promotion Education

A degree in Health Administration or Education is also accepted Undergraduate degree in: social work, nursing, physical/occupational therapy Competencies

Demonstrates leadership skills Communication, writing, presentation and interpersonal skills. Demonstrates word processing skills using Microsoft Office programs Knowledge and Abilities

Competent critical thinking/judgement/autonomous decision making capabilities Demonstrates the ability to have a meaningful Goals of Care discussion Work collaboratively w/ a multidisciplinary team&other agencies in the community Demonstrates ability to apply research and theory to practice Demonstrates strong commitment to "patient centered care" Knowledge in LEAN management system Demonstrated initiative, self-direction, organizational&leadership abilities Develop/present educational training material to healthcare professionals Knowledge in population health promotion approaches and community development Coping skills to work w/persons dealing w/stress&other end of life issues Other Information
