Job Summary We are seeking a dedicated and detail-oriented Clinical Prescribing Pharmacist to join our healthcare team. The ideal candidate manages acute minor ailments and chronic conditions through direct patient assessment, independent diagnosis, and prescriptive authority. Key Responsibilities Clinical Assessment & Diagnosis - Conduct Assessments: Perform focused physical assessments and comprehensive patient interviews to evaluate presenting symptoms. - Formulate Diagnoses: Diagnose designated minor ailments, minor acute conditions, and chronic diseases within the legal scope of pharmacy practice. - Order Diagnostics: Order and interpret relevant laboratory tests to safely monitor therapy, rule out differential diagnoses, or confirm clinical indications. - Triage Patients: Recognize red-flag symptoms and promptly refer complex, systemic, or high-risk cases to emergency services or primary care physicians. Prescriptive Authority & Medication Management - Prescribe Medications: Initiate, adjust, renew, or de-prescribe medications and vaccines safely based on clinical diagnosis and evidence-based guidelines. - Review Histories: Analyze complete medication histories to identify drug-related problems, potential interactions, or adverse drug events causing current symptoms. - Design Care Plans: Develop personalized, measurable therapeutic plans incorporating both pharmacological interventions and lifestyle modifications. - Monitor Outcomes: Schedule and conduct follow-up assessments to evaluate treatment efficacy, patient compliance, and potential side effects. Interprofessional Collaboration & Education - Coordinate Care: Maintain seamless communication with family physicians, specialists, and community health teams regarding patient care plans and prescriptions. - Counsel Patients: Educate patients on their diagnoses, correct medication administration, expected outcomes, and when to seek further medical attention. - Document Care: Document all patient assessments, rationales for diagnosis, lab orders, and prescriptions accurately in shared electronic medical records (EMR). Qualifications and RequirementsEducation & Licensure - Degree: Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy from an accredited institution. - Licensure: Active, unrestricted pharmacist license in positive standing with the College of Pharmacists of British Columbia. - Certifications: Valid certification for Independent Prescribing or Additional Prescribing Authorization (APA). - First Aid: Current CPR and First Aid certification. Skills & Knowledge - Clinical Knowledge: Deep understanding of pathophysiology, pharmacology, and therapeutics across minor ailments and chronic diseases. - Diagnostic Skills: Proficiency in taking clinical histories, performing basic physical assessments, and interpreting laboratory data. - Communication: Exceptional verbal and written communication skills for patient counseling and interprofessional collaboration. - Software Literacy: Experience working with pharmacy management systems and Electronic Medical Records (EMRs).
