Description

The successful applicant will work in compliance with relevant legislation and Hospital Policies and Procedures, practice the Bluewater Health Standards of Performance Excellence, and fulfill their role within the Attendance Support Program parameters. The successful applicant will possess effective communication and interpersonal skills, the ability to work independently and as part of a team, and strong attention to detail and accuracy.

Qualifications

Graduation from a Faculty of Pharmacy of a recognized university (required)

Licensed by the Ontario College of Pharmacists– Part A (required)

Doctor of Pharmacy (Pharm D) degree (preferred)

Accredited Canadian Pharmacy Residency (ACPR) certificate (preferred)

Skills, Ability and Experience

Proficiency using the Microsoft Office Suite (required)

Previous experience with Pharmacy software such as Meditech, OPIS, Kroll, Pyxis (preferred)

Previous experience in a Hospital Pharmacy (preferred)

Experience in Oncology pharmacy (preferred)

Experience in Mental Health pharmacy (preferred)

Previous experience in a Retail pharmacy setting (asset)

All staff pharmacist duties, including oncology or mental health roles in the outpatient pharmacy (asset)

Responsibilities

Review medication orders for technical accuracy to ensure authenticity, accuracy, and appropriateness.

Assess physicians' medication orders for therapeutic appropriateness, detect duplicate therapy, adverse reactions, drug‑disease incompatibilities, significant interactions, and recommend alternative drug therapy or pharmaceutical care plans.

Monitor patient response to drug therapy for effectiveness and adverse effects, recommend changes in therapy when necessary, and monitor lab results and other clinical indicators.

Apply antimicrobial stewardship to all antimicrobial orders, including therapeutic drug level monitoring.

Consult with and advise medical staff on drug utilization, availability, indications, dosages, warnings, incompatibilities, and contraindications.

Counsel patients and families on medication use, dosage, side effects, and correct administration techniques.

Provide direction and guidance to Pharmacy Technicians.

Enhance professional growth by participating in educational programs, current literature reviews, in‑service meetings, and workshops.

Participate in multidisciplinary rounds, hospital committees, and other meetings as required.

Participate in unit/department and Hospital Quality Improvement initiatives, projects, and order set reviews.

Complete continuing education requirements as necessary.

Perform other related duties as assigned.
