Position Overview

Sign on Bonus $2500 In this community nursing role, you, as a Registered Practical Nurse (RPN), will conduct scheduled visits within the community to provide care for patients in various settings, with support from RNs for more complex patients. You will work with patients of all ages with varying complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.We Offer

Participate in scheduled community visits to address patient needs within your scope of practice Provide ethical and safe nursing care Use appropriate assessment tools and techniques to evaluate patients’ individual needs Execute and continuously monitor care plans, adjusting as required Engage patients collaboratively by building rapport and trustMaintain and update accurate patient records related to encounters and plans What You Bring

Registration with the CNO as a Registered Practical Nurse in good standing Valid driver’s license and access to a vehicle Experience delivering community care is an asset Reliability, flexibility, and willingness to travel within a designated area Experience working with vulnerable populationsPassion for making a difference in the community Availability for on‑call coverage, including after hours, to ensure patient safety Compensation
