Position: Nurse Practitioner (Primary Care Team Westgrove Clinic) Westgrove Clinic works in support of the Patients Medical Home Model with a mission to support family physicians in improving primary care and access through relationships, collaboration and innovation. Westgrove Clinic is a group of family doctors who work with other health professionals to coordinate the comprehensive delivery of primary care services for their patients. Our team of health professionals include Nurses, Diabetic Educators, Dieticians, Pharmacists, Behavioural Health Consultants and Social Workers who collaborate to provide comprehensive primary care services for the patient population. Employment type: Contract position commencing March 15, 2023, renews annually. 0.6-0.8 FTE (hours) available. Please state your available weekly hours on your application. Location: Westgrove Clinic will be the primary location where you will be working alongside our member-Physicians. Objective: The Nurse Practitioner should be comfortable to liaise and consult with physicians as part of the care team, and to perform comprehensive assessment, diagnosis, intervention, management, referral, and evaluation of patient care across the continuum of health care services. Roles and Responsibilities · Works within the full scope of practice and code of ethics as delineated by the College & Association of Registered Nurses of Alberta (CRNA). Collaborates with an interdisciplinary team to safely and effectively provide appropriate primary health care to manage the primary health care needs of all Westgrove Clinic’s patients. - Conducts individual health assessment, diagnosis, treatment, follow-up and chronic illness monitoring; immunizations, orders diagnostic and screening tests, provides health promotion and disease prevention counseling - Identifies and provides appropriate treatment and/or referral for patients’ physical and psychosocial health problems in accordance with any current medical directives - Shares results of assessment and intervention with other IHPs when appropriate - Works with collaborating physician for issues falling outside the NP scope of practice - Instructs patients on specific diagnostic tests and therapies - Implement goal-oriented, time-limited intervention plans, including education in collaboration with patient and/or family and inter-disciplinary team - Applies self-management and other strategies to support the development of an informed patient - Evaluates and documents outcomes of patient care in collaboration with the team - Participates in staff meetings, program development, and improvement activities · Maintains and develops competence through ongoing continuing education including self-directed learning - Develop, update and maintain education resources Previous Experience: - Three to five years of experience in the Nurse Practitioner role - Previous experience in primary care setting is preferred - Chronic disease management - Demonstrated experience using EMR and Netcare (Accuro experience preferred) Qualifications: - Bachelor’s Degree in Nursing is required License/ Certification(s): - Active skilled registration with the College and Association of Registered Nurses of Alberta (CRNA) and registered on the CRNA Nurse Practitioner roster. (Must be able to provide proof of registration for all-ages) - Basic Cardiac Life Support (BCLS) Personal Attributes: - Enthusiasm and passion for providing quality care - Adaptability - Take initiative and accountability for delivering the best primary care to our patient population - Excellent oral and written communication skills - Demonstrated ability and willingness to work in a rapid environment that requires flexibility and balancing of competing priorities - Ability and desire to develop new skills and evolve with a continuous improvement focus - Being able to effectively communicate with the team.
