Reporting to the Executive Director, the Social Worker provides comprehensive psychosocial services that promote the social and cultural well‑being of residents in accordance with the Fixing the Long‑Term Care Act, 2021, and qualified standards. This role supports resident‑centered care, facilitates admissions and transitions, promotes rights advocacy, and contributes to interdisciplinary planning and quality improvement.Key Responsibilities and Skills
Complete psychosocial assessments on admission and periodically thereafter. Develop individualized psychosocial care plans based on resident needs. Provide therapeutic interventions, including grief and trauma support. Coordinate with the Admission Matters team, HPG, and external partners to support pre‑admission reviews.Educate families about LTC services, programs, and rights. Lead psychosocial aspects of discharge planning and follow‑up. Advocate for residents’ rights and support ethical decision‑making. Act as a mediator in resolving resident/family concerns. Report and document suspected abuse or neglect as per RHPA and FLTCA.Actively participate in care conferences and team rounds. Liaise with community agencies and internal departments. Communicate using a trauma‑informed, culturally inclusive approach. Maintain accurate, timely documentation in accordance with PHIPA and OCSWSSW standards. Assist with Ministry compliance audits and inspections.Lead psychosocial programming for resident engagement and emotional support. Contribute to data‑informed quality improvement initiatives. Remain current with best practices and changing regulations. Qualifications
Bachelor or Master of Social Work (BSW/MSW) from an accredited university Registered and in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) Minimum 2 years of experience in long‑term care or related healthcare setting Training in GPA, trauma‑informed care, grief counseling, or CBT is an assetExcellent interpersonal, organizational, and documentation skills Compassionate communication and emotional intelligence Understanding of aging, dementia, and mental health in older adults Commitment to inclusion, equity, and resident‑centered care Ethical decision‑making and advocacy Knowledge of relevant legislation: FLTCA, PHIPA, RHPA, AODA, OHSAProof of full immunization (2 doses) against COVID‑19 Employment Information
Job Types: Full‑time, Permanent Pay: $65,000.00-$70,000.00 per year Education: Bachelor’s Degree (preferred) Experience: Social Worker: 3 years (preferred), Long Term Care: 2 years (preferred) Licence/Certification: Ontario College of Social Workers Registration (required) Work Location: In personEqual Employment Opportunity
