(Step 1‑7 of a seven‑step salary grid; candidate experience and internal equity will be considered when determining the starting salary.) Key Responsibilities Manage the full accreditation cycle as CMHA‑YRSS Accreditation Coordinator. Implement and coordinate accreditation surveys, tools and instruments. Interpret accreditation results, develop action plans and remedial actions. Embed accreditation criteria into daily operations for ongoing preparedness. Train and coach staff on accreditation processes and requirements. Lead the interpretation of QI and patient safety education, coaching and mentorship. Develop tools, templates and resources to support quality improvement work. Provide risk management support and advice for high‑risk situations, complex cases and patient safety matters. Facilitate use of risk management frameworks such as Root Cause Analysis, Failure Mode & • Effect Analysis, safeguard analysis, Lean, Six Sigma and project management. Engage staff, management, clients, families, partnership organizations and other stakeholders in improvement initiatives. Develop staff to lead and coach others on quality improvement tools and practices. Provide project management for select QI projects. Represent CMHA YRSS in CMHA Ontario’s Quality Leads group and other working groups. Support VP, Clinical Operations & • Director in environmental scans for strategic planning. Expand the client/family voice in the mental health/addiction system. Draft policies and procedures. Support the Board‑level Quality & • Safety Committee and Client & • Family Advisory Committee. Collaborate with Health and Safety coordinator on safety plans. Complete reports for stakeholders and senior leaders. Lead the corporate incident reporting process, manage and analyze incident data and provide recommendations. Identify and communicate potential risks from incident report trends and propose mitigation strategies. Requirements Bachelor’s degree or diploma from a college or university. Regulated health professional with registration in good standing (preferred). Continuing education in quality improvement (root cause analysis, client safety, client engagement, client experience, quality improvement plans, dashboard development). Formal education in incident and risk management. Lean Six Sigma certification (asset). Canadian Risk Management designation (asset). Formal education in project management or PMP designation (asset). Prior Work Experience Five years or more in quality improvement, patient safety and risk management, including development of quality scorecards and evaluation of quality data. Facilitated accreditation processes. Experience in incident management. Experience in clinical risk management. Experience working in a community setting. Project management experience (asset). Knowledge Extensive acquisition of the accreditation process, ideally with Accreditation Canada. In‑depth knowledge of Ontario health care agenda. Strong understanding of continuous quality improvement, patient safety and risk management practices, client/family experience tools. Knowledge of legislation and regulatory workplace: Excellent Care for All Act, Quality of Care Information Protection Act, Privacy Act, Mental Health Act, PHIPA, Occupational Health and Safety Act, Ontario Human Rights Code. Additional Requirements A valid Canadian driver’s license. Minimum $1,000,000 third‑party liability insurance. Working Conditions Extensive desk work using computer and telephone for up to 7 hours per day in a standard open office setting. Weekend and evening hours may be required.
