Responsibilities
Develop and modify the care plans in response to residents’ individualized health care needs and changes in health status.
Liaise with other health care personnel, agencies, and families.
Maintain good relationships with team members, residents, families, and the general public.
Engage in meaningful interactions with residents during your entire shift.
Supervise all activities on the unit and take charge as required.
Monitor performance and complete performance evaluations for the Personal Support Workers (PSWs).
Direct, assign, and evaluate the work of the PSWs.
Be in charge of the home as required; utilize leadership skills to identify problems and solve them on the unit.
Ensure care is consistently delivered in accordance with the resident Bill of Rights.
Understand the nature and meaning of quality indicators used by the home and participate in improving these indicators to achieve satisfactory results.
Actively listen to resident and family concerns and take appropriate action to resolve issues in accordance with home policies.
Transcribe and process physician's orders for medication or other treatment in accordance with CNO standards.
Order and ensure nursing supplies and equipment are properly and efficiently used.
Conduct unit rounds at the beginning and end of shift.
Administer medications and treatments in compliance with the policies of Pharmacy provider, the Home, and the Standard of the College of Nurses.
Document and report all errors and omissions.
Complete and ensure residents' clinical care assessments, care plans, and documentation are accurate, up-to-date, and in compliance with CNO standards.
Communicate changes in health status to the Substitute Decision Maker (SDM) and interdisciplinary team.
Attend and participate in resident care conferences as required.
Maintain accurate and complete records of nursing observations and care in compliance with legislative requirements and home policies.
Assess changes in health status and report changes to the SDM and interdisciplinary team.
Complete assessments (RAI-MDS and other assessments), develop and update care plans for each resident in collaboration with the resident and/or SDM and the interdisciplinary team.
Inspire others to be motivated and work with passion, empowering team members.
Perform any and all other tasks as assigned.
Qualifications
Current Certificate of Competence with the College of Nurses of Ontario as a Registered Practical Nurse or equivalent registration in another Canadian jurisdiction, with authorization to practice independently in Ontario.
Basic CPR certification (asset).
Experience in long-term care setting (asset).
Ability to work with and relate to interdisciplinary team members.
Ability to work from the heart and make others smile.
Well-developed written and oral communication skills.
Knowledge of RAI-MDS; AIS certified, or must complete certification within one year of hire.
Clear vulnerable sector screen no older than 6 months.
TB test results no older than 6 months.
Three supervisory references.
