Social Work CCC - 0571.470000012 Job Category: Complex Continuing Care Length of Contract: 12 months Contract Type: Temporary Union Group: Non Union - Allied Health Schedule: Days 8 hours Employment Type: Part Time The Social Worker will report to the Clinical Services Manager, Functional Enhancement Unit, and will be part of a multi‑disciplinary team providing care to patients on the Functional Enhancement Unit. Responsibilities Support the needs of the social work team. Demonstrate skilled accountability, strong teamwork skills, and a willingness to participate in quality improvement projects related to the unit and the organization. Travel between and work at all Osler sites as required. Qualifications Three to five years of experience working with hospital or related settings. Knowledgeable in community resources. Eligible for OCSWSSW membership. Knowledge and experience working with relevant legislation (e.g., SDM Act, POA, Consent and Capacity). Knowledge of the psychosocial impact of chronic illness on the person and the family. Experience with MDS and FIM scores. Experience with post‑stroke population. Excellent communication and interpersonal skills. Ability to work as part of a multi‑disciplinary team. Excellent organizational skills and ability to set priorities as required.
